Birth Mark Referral & Intake

Birth Mark cannot accept self-referrals. We ONLY accept referrals from referring partners (including healthcare providers public health nurse, social workers and social service organizations). If you woul like to become a referring partner please email info@birthmarksupport.com

If you qualify to submit a referral, please complete the form below. Please ensure that you have reviewed the Birth Mark Client Criteria prior to referring your client.

Toronto Birth & Postpartum Intake

While we endeavour to support as many clients as possible, our capacity for reproductive support providers is currently limited. Please ensure your client meets our eligibility requirements and fill out an intake form. Following submission, a coordinator will reach out for a brief phone call with the referring agency. Intake form submission does not guarantee support availability.

*All workshops will be invoiced for $120 upon confirmation, tax receipts of $60 will be issued

Birth Mark Abortion Support Intake Links

Since there are many barriers to accessing timely and unbiased abortion support services, our Abortion Support Program does not have the same eligibility requirements.

The Seed & Sprout Community was created to address 2SLGBTQ+ specific barriers to access family building and reproductive supports. This is for 2SLGBTQ+ folks seeking care and support from queer and trans doulas.
